Cleveland Clinic Fitness Centers
For memberships to Walker, CCAC (Beachwood), BOC (Independence), Fairview, Hillcrest, Lutheran, Euclid, and Mercy Main Hospital only:
1. Click the button below to complete the online application. If you have already done this, go to step 2.You will need your Employee Health Plan ID number; number will start with a W.
2. Email fitness@ccf.org and let us know you completed the application. We will review your application and process your CC employee ID badge for access and/or provide you with a PIN number (you can use either).

4. Applications can also be processed in person at the Walker fitness center.
5. For Payroll or Self-Pay, memberships contact us at fitness@ccf.org or call 216.444.8765.
Akron General Lifestyles Fitness
For memberships to Lifestyles in Bath, Stow, Green and Express:
1. Click the button below to complete the online application. If you have already done this, go to step 2.You will need your Employee Health Plan ID number; number will start with a W.
2. Contact the location in which you will primarily go to for details to complete enrollment process.

3. Bring a copy of your registration or email confirmation along with your badge and/or photo ID.
Medina Community Recreation Center (MCRC)
1. Complete the online application. If you have already done this, go to step 2.
You will need your Employee Health Plan ID number; number will start with a W.

If you are a spouse or a dependent, you will need to be entered into the clinic security system. Please let me know if there are dependents registering, there will be additional information needed.

All family members interested in joining must register together.
3. Keep in mind there is a visit requirement attached to the MCRC membership. Inquire directly at the email above.
4. In conformity with current IRS rules and regulations, the value of your fitness center membership and other wellness programs are generally considered taxable income and reportable on your Form W-2. This applies to contracted vendors, such as Medina County Recreation Center.
