Cleveland Clinic Fitness Centers

For memberships to Walker, CCAC (Beachwood), BOC (Independence), Fairview, Hillcrest, Lutheran, Euclid, & Wooster:

1. Click the button below to complete the online application.  If you have already done this, go to step 2.You will need your Employee Health Plan ID number; number will start with a W.  

2. Email fitness@ccf.org and let us know you completed the application.  We will review your application and process your CC employee ID badge for access. 

  • If you are a spouse or a dependent, you will need to obtain a fitness center badge
    (if you do not already have one). Please let us know when you email us
    and we will follow up with further details.  
      

3.  When visiting a fitness center, you must bring either your employee badge or fitness center badge each time and swipe in at the card readers.

5. Applications can also be processed in person at the Walker fitness center.
 

6. For Payroll or Self-Pay, memberships contact us at fitness@ccf.org or call 216.444.8765.

Akron General Lifestyles Fitness

For memberships to Lifestyles in Bath, Stow, Green and Express:

1. Click the button below to complete the online application. If you have already done this, go to step 2.You will need your Employee Health Plan ID number; number will start with a W.  

2. Contact the location in which you will primarily go to for details to complete enrollment process.

3. Bring a copy of your registration or email confirmation along with your badge and/or photo ID.

 

Medina Community Recreation Center (MCRC) 

1. Complete the online application. If you have already done this, go to step 2.  
You will need your Employee Health Plan ID number; number will start with a W.  

2. Email Lindsey Sitko: sitkol@ccf.org to review your application and activate your CC employee ID badge and provide you with the documentation needed to activate your membership at MCRC directly. 

  • If you are a spouse or a dependent, you will need to obtain a fitness center badge
    (if you do not already have one), please include that in your email. There will be a follow up email requesting additional information. A dependent badge is a requirement for the membership. 

  • All family members interested in joining must register together. 

3. Keep in mind there is a visit requirement attached to the MCRC membership. Inquire directly at the email above. 

4. In conformity with current IRS rules and regulations, the value of your fitness center membership and other wellness programs are generally considered taxable income and reportable on your Form W-2.  This applies to contracted vendors, such as Medina County Recreation Center.
